Yes. With a site license, we create a custom build for your facility that works on all your machines; meaning, unlimited seats of Fracture FX with no need for a license ever. Contact us for site license packages.

Most likely, you'll have to purchase a license transfer to get a license for the new machine. As mentioned in our terms and conditions: "Each license is for one node-locked seat. Transferring an existing license to another machine will incur a transfer fee. "
(To transfer a Fracture FX 1.x license, you can use this link.)
Depends on what hardware you swapped out. We suggest you transfer the license and the plugin to the rebuilt machine and try it out. If it doesn't work, you'll have to pruchase a license transfer.
As long as motherboard and network card didn't change, your license will work across the different operating systems. The installation package contains a build of Fracture FX for MacOS, Windows and Linux. It also contains builds for the most recent versions of Maya. If your motherboard or network card did change, you will need to purchase a license transfer.
This means your license is not installed, or not installed correctly. Have an other look at the installation instructions on the wiki.
You can render Fracture FX geometry on a (networked) machine without a license, but you can not simulate on a machine without a license (see FAQ about nodelocked licenses). To render on machine without a license, simply replace your simulation with keys, or bake it to alembic. Then import that geometry in your render scene as per usual. See our tutorial section for more in depth information.

No. We will not be releasing a build of Fracture FX for Maya 2016 ext2. The reason being that ext2 is binary incompatible with Maya 2016, meaning 2016 plugins will not work with Maya 2016 ext 2. This is something Autodesk decided to do and is out of our hands.
The solution is to do your Fracture FX sim in 2016, and then write to keys, or alembic. Then you can import those assets in Maya 2016ext2.
